Experiences Before and After Receiving Disease Specific Care Quality Certification for Traumatic Brain Injury: A Reflection Report

台北慈濟醫院於2023年7月成立創傷性腦損傷(Traumatic Brain Injury, TBI)照護團隊，致力於全人化與專業化醫療，並於2024年參與創傷性腦損傷照護品質認證。團隊涵蓋神經外科、護理、復健、藥事、營養與社工等專業，透過標準化流程與數位工具，如「住院病人出院後追蹤動態APP」監測康復進展，並推動醫病共享決策(SDM)以提升病人參與度。認證過程中，團隊檢視急診處理、手術照護與出院追蹤等環節，並根據委員建議，優化個案管理、資訊化系統與風險控制。認證後，TBI照護品質顯著提升，格拉斯哥昏迷量表(Glasgow Coma Scale, GCS)評估記錄達100%，CT檢查完成率提升至96.64%，再入院率下降。此次認證肯定了團隊的努力，也促進了品質改善與跨領域合作。我們將持續強化病人安全與照護品質，為每位TBI病人提供專業、安心且溫暖的醫療服務。

 

In July 2023, the Taipei Tzu Chi Hospital established a traumatic brain injury (TBI) care team dedicated to providing holistic and professional medical services. In 2024, this team received a TBI care certification. To promote interdisciplinary integration, the team includes neurosurgery, nursing, rehabilitation, nutrition, pharmacy, and social work specialists. The team monitors patient recovery through clinical procedures and digital tools, including a mobile app for following up on patients after their discharge, and patients and families are encouraged to exercise shared decision making regarding treatment. During certification, the team reviewed all stages of care, spanning emergency management and surgical care to post-discharge follow-up. Case and risk management processes and digital systems were improved on the basis of recommendations from expert reviewers. Patient outcomes markedly improved after certification: Glasgow Coma Scale scores reached 100%, computed tomography completion rates increased to 96.64%, and readmission rates decreased. The team received TBI care certification as recognition for its efforts, reinforcing a culture of continuous quality improvement and collaboration. The TBI care team remains committed to patient safety and care quality, striving to provide safe, professional, and compassionate medical services to all patients with TBI.
