Purpose: Previous studies have indicated case management models markedly improve care outcomes for patients with acute myocardial infarction (AMI). In this study, quality improvement methods were adopted to address the suboptimal enrollment rate observed in our hospital’s case management program, balance care capacity and quality, and boost AMI case enrollment rates and care outcomes. Methods: The research team investigated two key questions: why is the AMI case enrollment rate low, and how can AMI care outcomes be improved? A total of 14 root causes were identified, and 16 targeted interventions organized into 7 strategy groups under the AMI-CARE framework were implemented. Results: The AMI case enrollment rate increased from 16.8% to 62.5%, representing a target accomplishment rate of 138% and progress rate of 272%. All seven secondary objectives were also met. Conclusion: The AMI-CARE framework effectively improved AMI case enrollment rates and overall care quality. This approach can serve as a reference for other disease certification teams.
