提升急診護理師對毒傷病人照護之完整率

Improving the Completeness of Nursing Care for Poisoned Patients in Emergency Settings

快速辨別毒物種類並給予正確的處置是維持中毒病人生命最重要的一環。本單位回溯毒傷病人照護過程發現，急診護理師因照護不完整導致追蹤病程變化困難，同時影響照護品質，故成立此專案小組進行改善。小組於2022年1月至2月間藉由作業完整性觀察表及認知調查表發現同仁對毒傷病人照護完整率僅為66.9%，認知率為68.8%；進行討論與分析後藉由特性要因圖確立導因進行改善，小組透過規劃毒傷認知教育訓練、毒傷病人交班單、照護指引、擬定紀錄準則及設計多媒體數位教材等對策有效提升照護完整率至98.1%，認知率也提升至98%，成效顯著。透過本專案實施成效分享，提供其他醫療院所等相關單位毒傷病人照護完整性改善之參考。

Quick identification of the causative toxic substance and immediate, correct management play a crucial role in saving lives, yet a retrospective review revealed that incomplete nursing care compromised patient tracking and overall care quality. To tackle this issue, a task force was established to analyze clinical workflows, discovering that from January to February 2022, nursing staff had a poisoning awareness rate of just 68.8% and an operational completeness rate of 66.9%. Through rigorous discussion and cause-and-effect analysis, the team designed and implemented a series of targeted interventions, including a comprehensive poisoning awareness training program, standardized poisoning nursing notes, specialized clinical care guidelines, strict recording standards, and interactive multimedia educational materials. These strategies yielded a dramatic improvement, successfully driving operational completeness up to 98.1% and nursing awareness to 98%. Ultimately, the success of this project demonstrates that structured educational resources and standardized documentation can serve as a promising, replicable model for improving the quality and safety of emergency poisoning care across other healthcare institutions.
