This describes an 81-year old male cerebrovascular accident patient who accepted discharge planning,went home process of the attendance. The nursing period was from August 15 to September 2, 2006. Theauthor utilized discharge planning, the Barthel's score and Gorden 11 functions to carry on the appraisal, andmain health questions which included impaired wheelchair mobility, risk of falls, self-care deficit, and disturbedbody image. The nursing process, assisted in facing the loss of energy, accepting his body, heart recoverytreatment, and the promotion of body function. It also and improved self-attendance ability reconstructionof self-concept, and helped the patient to overcome the bodily image disorder. It is hoped this experience canassist staff in treating similar patients and reduce the physiology of the illness. |
