蜂螫及過敏性休克病患的護理個案報告

A Case Report on Nursing A Honeybee Stung Patient with an Anaphylactic Collapse

本個案報告是描述一位47歲病患，兩年內曾有兩次蜂螫經驗，而此次發生嚴重過敏性休克；到院時已無生命徵象，經急診醫護人員緊急處理，正確快速的評估，並給予各種急救處理與護理措施，而恢復病患之生命徵象。此休克病患雖已給予最適當的急救與治療，但因病發至到院的時間超過4-6分鐘急救的黃金時段，仍造成病患某種程度的損害，如腦部缺氧之後遺症等。因身為急診醫護人員，對於過敏性休克問題的嚴重性更應有深切體認，以減少對病患造成的損傷，希望藉由蜂螫及過敏性休克病例的探討，對提供病患的緊急救護上有所貢獻，亦是本文的精神所在。

A 47-year-old man was delivered to the emergency department with no vital signs after an anaphylactic collapse resulting from a honeybee sting. He has experience a bee stinging twice within two years. Although emergency resuscitation was provided as soon as possible, his basic vital signs were restored. Unfortunately, irreversible brain damage due to hypoxia was believed to occur after 4 to 6 minutes of initial delay in golden period of emergent resuscitation, even though we did give him duly and proper treatment. We, as emergency department staff, should pay attention to the systemic reactions to honeybee stings and the pathophysiology of the anaphylactic shock. We hope we can understand better and learn more about anaphylactic shock through this case.
