Emergency Nursing Experience of Caring for a Foreign Spouse Suffering from Domestic Violence

本文描述一位遭受先生暴力相向的越南籍配偶至急診就醫的護理經驗，就醫日期為2016年1月18日，主訴多次遭受先生暴力毆打，本次因為頭部外傷至醫院求治。從檢傷開始發現個案坐立不安且焦慮無助，身旁無親友陪伴，運用Watson關懷理論於照護期間建立良好護病關係，安排隱密、安全環境進行醫護處置，協助驗傷採證及通報流程，經由觀察、傾聽、會談來收集資料，確立個案有：皮膚完整性受損、恐懼及個人因應能力失調等健康問題。於陪伴及關懷中鼓勵個案表達內心感受及情緒宣洩，提供家暴相關社會資源，並協助轉介相關防治單位以提供專業及妥善的照護，建議未來急診教育訓練中可擴增保護性個案照護之相關課程，建置標準作業流程，讓同仁接觸到類似保護性個案，有規則可依循，也提供急診護理同仁們於日後照護保護性個案之參考。 This article describes an emergency nursing experience of caring for a Vietnamese woman from multiple domestic violence injuries. The patient was admitted to the emergency room on January 18th, 2016, expressing that she has had several instances of domestic violence with resulting injuries. The patient was admitted to the hospital in this particular instance due to an injury to the head, with initial assessments showing an anxious state and uneasiness with no friends or family around for support. The author applied Watson's caring theory during the nursing care period, and built a trusting relationship with the patient while also providing a safe and secure environment during injury assessments. The police were notified after a collection of evidence of domestic violence. Data was collected through observations, listening and interviews to identify the following nursing issues: impaired skin integrity, fear, and a lack of effective individual ability to overcome struggles. In addition to medical treatment, we also provided physical and psychological support through companionship and care to ease the patient's mood and relieve her of her fear. Domestic violence counseling and social resources were also provided to help transfer her to other organizations that can provide her with necessary and complete resources to improve her situation. We strongly suggest that the training for emergency nursing care should include instructions on how to care for vulnerable patients needing protection, and building a standard process for such cases. It is with hope that this article can be a reference for future cases when attending to patients experiencing domestic violence.
