一位手部外傷病患接受顯微重建之護理經驗

Nursing Care Experience for a Hand Crushing Injury Patient Receiving Microsurgical Reconstruction

本文是探討一位病患因工作傷害導致左手外傷，接受顯微重建手術的護理經驗。於照顧期間利用會談及觀察收集資料，以Majority Gordon十一項健康功能型態評估個案的問題，結果發現個案有組織完整性受損、自我照顧能力缺失、睡眠型態紊亂、及焦慮等護理問題。藉由傷口護理、自我照顧訓練、集中護理、及建立良好人際關係等措施，促使個案傷口逐漸癒合並接受截肢的事實，藉此護理經驗的分享，冀能提供日後護理同仁照顧類似個案時的參考。

This case study reported the nursing experience for a left hand crushing injury patient receiving microsurgical reconstruction. Four major nursing problems were identified by using Majority Gorden’s functional health assessment guide through interview and observation during nursing care period. They were impaired tissue integrity, deficit of self-care ability, disturbance of sleep pattern, and anxiety. Nursing intervention programs including wound care, self-care training, focused nursing care, and establishment of better relationship with patient, were provided to improve wound healing and to assist acceptance of amputation event. This experience provided nursing staff with a guide to care similar patients in the future.
