心衰竭併活動無耐力之案例分析

Heart Failure with Activity Intolerance--A Case Analysis

本文描述一位心衰竭病患因急性肺水腫入院，在病情控制後因活動無耐力之問題而困擾個案，且臨床工作同仁對心臟運動復健亦半知半解，未能配合個案的狀況，即時給予合適的復健計畫與運動處方，讓心衰竭患者病情穩定出院後因身體活動無耐力，降低社交功能，衝擊返家後的生活品質，甚至反覆再入院。有鑑於此，本文藉由一位心衰竭之案例分析，透過提供患者漸進性的心臟運動復健之經驗，闡明早期計畫心衰竭運動處方之重要性。另外，筆者在協助此案例執行心肺復健過程中，認為個案的動機與高度的配合性是此案例成功的最大主因，因此醫護人員與患者身旁支持者所給予的鼓勵更是重要，期望藉此經驗可做為臨床護理同仁之參考。

We described a case of recovered heart failure with acute pulmonary edema being disturbed by activity intolerance. Most clinical practitioners are not familiar with cardiac rehabilitation as well as designing of appropriate rehabilitation plan and exercise program. As a result, this patient had inadequate social function and decreased quality of life due to physical activity intolerance after being discharged. Through case analysis, we have successfully introduced an appropriate exercise rehabilitation program. We emphasized the importance of early exercise prescription to heart failure patient. Furthermore, in addition to family and professional support, patient’s motivation and compliance were the major keys to the success of the cardiac rehabilitation plan.
