Global healthcare systems are entering a new era driven by artificial intelligence (AI), health information interoperability, and digital governance. Smart hospitals are no longer defned solely by the deployment of information technologies; rather, they represent comprehensive transformations encompassing clinical governance, data standardization, process redesign, cross-system integration, and international brand development. This article explores how Taiwan can establish a globally competitive smart hospital system from the perspectives of organizational capability and international influence. First, it examines the transformation of smart hospital governance models, illustrating how digital departments have evolved from traditional operational support units into strategic drivers of hospitals development. Second, it discusses the concept of end-to-end integration in smart healthcare and the application of AI in acute and critical care settings. Third, it explained how international standards, including Fast Healthcare Interoperability Resources (FHIR), Logical Observation Identifiers Names and Codes (LOINC), and Systematized Nomenclature of Medicine—Clinical Terms (SNOMED CT), from the foundation of Taiwan’s next-generation healthcare information infrastructure. Finally, it highlights the importance of international visibility, academic dissemination, and external validation in strengthening the global presence of smart healthcare institutions. This article argues that the competitiveness of smart hospitals depends not only on technological advancement but also on effective governance, international collaboration, and workforce development. Strengthening these capabilities will provide Taiwan with an important opportunity to become a global leader in smart healthcare.
