Down syndrome screening often presents communication challenges because the condition is complex and outpatient consultation time is limited. To ensure that information is understandable to patients, this study adopted service design principles and human factors engineering to develop a clear visual patient decision aids and a structured shared decision making(SDM) workfow. This approach enabled pregnant women to better understand available screening options and actively participate in decision making. Following implementation, understanding of screening diferences among pregnant women increased from 65% to 92%, and decision satisfaction improved from 3.8 to 4.6. For healthcare providers, the optimized decision aid and integration of digital access via QR codes reduced the need for repeated explanations, allowing for more focused and efcient discussions. This study demonstrates that a systematic SDM strategy can substantially enhance communication quality in perinatal care, strengthen patient–provider trust, and support patient safety and care quality.
