Purpose: To enhance kidney disease care, National Taiwan University Hospital has established a dedicated kidney care team focused on patient-centered health promotion. Object: The target population was the general public and all hospitalized patients with early-stage chronic kidney disease, acute kidney injury, pre-end-stage renal disease, and end-stage renal disease. Methods: The kidney care team adopts a patient-centered approach and a cross-disciplinary collaboration model. By participating in the Disease Specific Care-Kidney Disease (DSC-KD) certification program, which is managed by the Joint Commission of Taiwan, the team implements standardized procedures and promotes continual quality improvements in areas such as system development, clinical care, health education, health literacy, and community outreach. Results: In October 2024, the team again joined the DSC-KD certification program, reaffirming its commitment to excellence in kidney care. Conclusion: Diversified integration and innovation are key to improving the quality of kidney disease care. In the future, the team plans to integrate advanced information technology to further optimize care models with the goal of safeguarding public health and setting a benchmark in kidney care.
