住宿式照顧機構收托費用差異性分析：與機構規模屬性之關聯

Disparity Analyses of Monthly Care Fee of Residential Care Facilities: Relations With Institutional Size and Ownership

目的：瞭解住宿式照顧收托費用情形，並分析該費用與不同機構規模及屬性之相關性。方法：以臺北市為研究場域，蒐集政府公開長照相關次級資料，於2021年10月至12月間調查機構收托費用現況。運用描述性統計、推論性與多變量統計，以規模與屬性進行差異比較及檢定。結果：臺北市月平均收托費用35,814元，中位數35,000元。規模50床以下之小型機構每月平均收托費用34,928元、超過50床之大型機構41,184元，大型機構收費高於小型機構。屬性方面，財團法人與醫院附設機構收托費用皆高於私立小型機構。以t檢定和單因子變異數分析進行收托費用與機構規模及屬性之相關性檢定，發現不同規模收托費用有顯著差異（p = .026），規模50床以下收費低於超過50床。社政體系和衛政體系在收托費用上有顯著差異（p < .001），社政體系機構收費低於衛政體系機構。營利與非營利機構在收托費用上有顯著差異（p = .004），營利機構收費低於非營利機構。羅吉斯迴歸分析顯示，非營利機構收取高收托費用的機率是營利機構的17.042倍。結論：屬性和規模與收托費用相關。建議政府應明定收托費用組成結構，健全各類住宿式機構之照護品質發展。

Purpose: This study aimed to examine the situation of monthly care fee of residential care facilities in Taipei City, and to analyze the disparity between the fees and the size and ownership of these institutions. Methods: The data was collected from public released on the website and related governmental data in this study during October to December 2021. The statistics methods include descriptive analyses, single variate analyses and multivariate analyses to describe, compare and identify the associated factors with residential care fee among the residential care in Taipei City. Results: The average monthly care fee of 113 residential care facilities in Taipei is NTD 35,814 (median is NTD 35,000). With regard to the relation of institutional size and the monthly care fee, findings show that the care fee is NTD 34,928 in small size and NTD 41,184 for the large size. Those large size institutions seem more expensive than the small size. In term of the ownership to the monthly care fee, we found that those incorporated foundation and hospital affiliated institutions are significant higher price than those of small private settings. Finally, this study found that institutional size has significant differences in monthly care fee in t-test analyses (p = .026) and the ownership for-profit (FP) and not for-profit (NFP) institutions has significant differences in monthly care fee (p = .004) with FP institutions charging lower fees than NFP institutions. And the NFP are 17.042 times more likely to charge high monthly care fee than FP analyze under Logistic regression. Conclusions: This study highlights that size and ownership are related to monthly care fee in Taipei care facilities. We suggest that the government should provide the precise price information in residential care services and monitor the quality care in residential care settings.
