Infertility, which is defined as failure to conceive after 12 months of unprotected intercourse, affects 8–12% of couples worldwide. Tubal factor infertility (TFI), which is often caused by pelvic inflammatory disease and tubal obstruction, accounts for up to 35% of female infertility cases. Standard diagnostic methods include hysterosalpingography (HSG) and treatments typically involve surgical tubal repair or assisted reproductive technologies (ART), such as in vitro fertilization (IVF). However, the success rates of ART are poor so adjunctive therapies are an alternative treatment option. This case report describes a 30-year-old Taiwanese woman with bilateral tubal obstruction and a history of repeated implantation failure and miscarriages following IVF cycles. Traditional Chinese Medicine (TCM) treatment, using a diagnosis of cold coagulation with blood stasis, was administered using herbal medicine and acupuncture. The patient’s symptoms of dysmenorrhea and menstrual abnormalities improved and she subsequently achieved a natural pregnancy, which was confirmed by positive home pregnancy test, and delivered a healthy infant. Although post-treatment imaging was not performed to objectively confirm tubal patency, the spontaneous pregnancy supports the functional restoration of tubal function. Evidence from previous studies shows that Chinese herbal medicine and acupuncture have an anti-inflammatory and antimicrobial effect, which may contribute to the reduction of tubal inflammation, the prevention of adhesions and improvement in tubal patency and pregnancy outcomes. This case shows the therapeutic potential of integrative TCM methods for tubal infertility and demonstrates the importance of individualized pattern differentiation in treatment. Further controlled studies are warranted to confirm these findings and to determine the underlying mechanisms. |
