This study reports the first case of Phaeoacremonium parasiticum fasciitis in Taiwan. Initially, the fungus was misidentified as Exophiala, based on morphological characteristics at Hospital A. However, subsequent gene sequencing at Hospital B accurately identified the organism as P. parasiticum and antifungal susceptibility testing was also performed. At Hospital A, seven surgical debridements and treatment with fluconazole were attempted, but these efforts resulted in failed treatment. Only a single surgical debridement was performed and itraconazole was administered at Hospital B, which yielded a successful outcome. This study suggests that for the treatment of rare fungal infections, the fungus must be correctly identified and the results of antifungal susceptibility testing are necessary. |
