本文報告2025里斯本WONCA(World Organization of Family Doctors)世界大會舉辦「亞太家庭醫學最新進展研討會」,回顧澳洲、中國大陸、台灣、日本、菲律賓、印尼六國家庭醫學的發展現況、挑戰與社區照護中的革新,以為參考。
各國進展重點如下:澳洲制度成熟但面臨人力短缺與財務壓力,正推動論質計酬、數位轉型與擴大健保總額;中國大陸推行家庭醫師簽約服務以提升基層利用率,惟面臨品質與區域差異之挑戰;台灣透過法規保障病人自主權,在安寧緩和醫療展現高品質與國際影響力,但在非癌照護與人力上仍有進步空間;日本因應超高齡社會,致力於推動「30分鐘生活圈」的社區整合照護系統與跨專業團隊訓練;菲律賓與印尼則皆透過國家法令(如《全民健康法》、《2023年健康法》)強化家庭醫師在社區的主體性,並致力於改善訓練體系與醫療資源分布不均之問題。本文末綜整各國經驗,提出對台灣家庭醫學發展之啟示。
英文摘要
This paper reports on the 'Symposium on Recent Progress in Family Medicine in the Asia-Pacific Region' held during the 2025 WONCA (World Organization of Family Doctors) World Conference in Lisbon. It reviews the current development, challenges, and innovations in community care across six countries/regions-Australia, Mainland China, Taiwan, Japan, the Philippines, and Indonesia-to serve as a strategic reference.
The key progress of each nation is summarized as follows: Australia possesses a mature system but faces health workforce shortages and financial pressures; it is currently promoting value-based funding, digital transformation, and the expansion of the Medicare budget. Mainland China is implementing family doctor contract services to increase primary care utilization, though it faces challenges regarding quality consistency and regional disparities. Taiwan has demonstrated high-quality palliative care and international influence through legal frameworks protecting patient autonomy; however, there is still room for improvement in non-cancer care and manpower distribution. Japan, in response to a super-aging society, is dedicated to promoting the ''Community-based Integrated Care System'' within a '30-minute living radius' and interdisciplinary team training. The Philippines and Indonesia are both leveraging national legislation (such as the Universal Health Care Act and the Health Law of 2023) to strengthen the central role of family physicians in the community, while striving to improve training systems and address the inequitable distribution of medical resources. Finally, this paper synthesizes these international experiences to provide insights for the future development of family medicine in Taiwan.
