Medical prescription is a very common intervention for long-term care. Long-term care for the aged, disabled or dementia, consideration needs to be given to appropriate and relevant outcome measures including rehabilitation, nutrition care, feeding and dysphagia care, mental health care, long-term bedridden patient care, and environmental security care. On the basis of the criteria provided by Beers, STOPP/START, PIM-Taiwan, PRISCUS, EU (7)-PIM list, NORGEP-NH, FORTA and other guidelines, pharmacists conduct drug regimen reviews for each long-term care resident to assess the appropriateness and the efficacy of drug therapies. This paper can be applied as a screening tool to identify potentially inappropriate medications in pharmacotherapy optimization and assessing and improving the quality of life indicators in long-term care. |
