提昇冠狀動脈繞道術後病患護理指導完整性

Promoting The Integrity of Nursing Instruction for The Patients after Coronary Artery Bypass Grafting Surgery

嚴重冠狀動脈心臟病患者的治療，近年來藉由冠狀動脈繞道手術的發展下，預後獲得改善，並提昇了他們的生活品質。而病患術後的恢復程度，護理指導佔很重要的角色。本心臟專科病房推行此專案，建立了冠狀動脈繞道術後護理指導標準規範，以提昇護理指導完整性與護理服務品質；增進術後患者自我照顧的知識與技巧及護理指導的滿意度。於2004年10月至2005年4月，制定護理指導標準手冊，對單位中60位施行冠狀動脈繞道術後患者進行個別護理指導。推行結果：護理指導完整性由59%提昇至87.2%，病患對護理指導滿意度由39.3%提昇至75%，達到專案目標。專案實施過程中，成功得到院方與病人的支持與肯定，目前正將此模式應用在其他心臟術後病人，建立合適的護理指導常規，更期望推廣至院內的其他單位。目前，相當多的護理同仁選擇進入校園再進修，期望加強自己的學歷與能力，而使得護理人員在專業的領域上有很大的進步，而不單只是求取學歷而已，學習的過程中同時也活絡了院際交流與合作的網路。筆者在在在職進修期間，成功的協助振興醫院心臟專科病房完成此一專案，使該院的護理品質得以提升，合作的過程對彼此來說是相當特別的一種學習經驗，在此也提供給各專業同仁分享。

The development of coronary artery bypass grafting makes patients of coronary artery diseaseobtain the improvement; nursing instruction occupies the very important role in recovery degree ofpatients. This ward of the cardiology department carries out this project, establishing the normal standardof nursing instruction to promote the integrity and the service quality of nursing instruction andalso to improve both the knowledge and the skill of self-attendance for after surgery patients and thesatisfaction degree of nursing instruction. The formulation of nursing instruction standard handbookhas been set up from October, 2004 to April, 2005. 60 after coronary artery bypass surgery patients ofthe unit carry out individually nursing instruction. Results: the integrity of nursing instruction by 59%enhanced to 87.2%, the satisfaction degree of nursing instruction for patients by 39.3% enhanced to75%, achieved the project goal. The project implementation process obtains both the hospital aspectand patients support and the affirmation successfully. At present this pattern applies after in other heartsurgery patients, establishing appropriate nursing instruction conventions and hope can popularize tothe hospital in other units.In recent years, many colleagues to take advanced courses again, strengthened between ownacademic degree and the special ability also relative detachable the network which hospital and thehospital exchanged,1enabled each other to have the very big progress, no longer only was for obtain the academicdegree. This cooperation achieved the re-promotion pattern of nursing quality successfully which is aquite worth experience to provide for fellow colleagues to share.
