Purpose: Stroke is one of the main causes of elderly disability in physical and mental health. The aim of this study is to investigate the influence of stroke history on health care utilization of the middle-aged and elderly adults. Methods: Subjects who completed the interviews of the 'Survey of Health and Living Status of the Elderly in Taiwan' in both years of 2003 and 2007 were included. According to their stroke history, these subjects were divided into three groups: 3207 subjects never have strokes, 170 subjects got strokes between the two years and 109 subjects got stroke before 2003. The paired-t test and the generalized estimating equations approach were used to analyze the changes and their differences of the health care utilization for the three groups. Results: After 4 years of follow-up, the middle-aged and elderly health care utilization increased. ‘new stroke group’ emergency & hospitalization utilization rates higher than ‘no stroke group’. ‘old stroke group’ health examination utilization rates higher than ‘no stroke group’ between 2003 and 2007. Conclusion: Aging for the middle-aged and elderly had a comprehensive health impact. The changing of stroke status induced physical health disability and acute medical care utilization increased. The study has roughly quantified the impact of strokes on older people and could offer substantial reference materials for planning the social welfare and allocating the resource of health care. |
