Collecting specimens is an important routine which requires zero errors during the entire procedure. Any misstep during the procedure can affect the results, and may lead to misdiagnosis and inappropriate treatment of patients. Other results include increased workloads for nurses and increased medical costs. This project was performed in a surgical ward to investigate labeling errors during the collection of specimens. Two concerns were raised during the investigation: the rate of mislabeling specimens was as high as 0.25% and the Standard Operating Procedure (SOP) for specimen collection was not performed accurately by all nurses. Bar code technology, an updated SOP, and an on-the-job training course were implemented. With these methods, the error rates for specimen labeling and SOP execution were reduced from 0.25% to 0% and from 27.4% to 2.7%, respectively. The improvement in specimen labeling as a result of this project may serve as a stimulus for other hospitals to implement bar code technology. |
