居家主照顧者與護理人員對遠距居家照護需求之認知差異

Comparison perception regarding telehome care between home care-giver and home nurse nurse

本研究主要目的在於了解居家主照顧者在遠距照護的認知感受，並分析比較居家主照顧者與護理人員對遠距居家照護服務內容之需求與差異。研究採方便取樣，取得中部地區五所教學醫院有接受居家照護服務之主照顧者211人及居家護理人員11人，進行配對211人次實際訪視，結果顯示：主照顧者有聽過遠距居家照護服務的有17.1%。目前現有的居家設備與使用能力九成以上為電視與電話，所以期望未來提供的照護方式是透過電話與電視。影響主照顧者是否使用新科技的因素包括機器本身是否容易學與容易操作、與健保是否有給付。主照顧者與居家護理師認為服務內容的需求比率皆在五成以上，且居家護理師針對居家病患認為其對各種照護內容的需求明顯高於主照顧者。兩者皆認為很重要的功能項目為：病患狀況記錄定期傳至醫療團隊、健康評估、血壓測量、緊急狀況設備能警示。期冀以此為基礎，提供資訊管理發展日後遠距居家照護軟硬體設計之參考。

The aim of this study was to demonstrate the home care-givers’ perception regarding telehome care (THC) .Furthermore, to compare the difference of THC need between home care-giver and home nurse. Convenience sampling was conducted, 211 care-givers and 11 home nurses were included of five teaching hospitals in middle Taiwan and there were matching 211 actual visits of care-givers and home nurses. The results showed there were 17.1% care-giver had ever heard about THC, above 90% could use television and telephone. They expected television and telephone as the THC pattern in the future. The equipments whether easy to learn or to operate, whether the reimbursement pay or not were the most important affecting factors to this new information technology in THC. Above fifty percent THC need in care-giver and home nurse,. Home nurse had significant high need than care-giver. They all thought it important function in patient record periodical transfer to health care team, patient physical assessment, blood pressure taking, and the alarm function when emergency happened. According to the evidence-based data, providing the reference of information management department to develop telehome care in the future.
