一位正位肝臟移植手術後病人之護理經驗

Nursing Experience of a Patient with Orthotopic Liver Transplantation

本文描述護理一位正位肝臟移植術後並接受免疫抑制劑治療的病患，由加護病房轉至普通病房後，免疫抑制劑服用治療到出院期間之護理過程。以羅氏適應模式為評估架構，確立個案主要健康問題有：身體活動功能障礙、家庭運作過程改變、需要護理指導。在護理過程與個案建立良好治療關係，給予生理症狀不適的照護外，筆者在醫療小組間扮演觀察、溝通、協調、諮詢、轉介等角色，使個案能於醫療團隊支持下順利出院，並能獲得良好的居家照顧。The purpose of this article was to explore the nursing experience of a female patient who received orthotopic liver transplantation and immunosuppressant treatment. Her condition was evaluated by “Roy’s Adaptation Model”. The assessment showed the main health problems as follows: impaired physical mobility, atlered family relationship, and necessity of caring education. During nursing period,the author with honest attitude created good therapeutic relationship with the patient. Besides caring symptoms of physical discomfort, the author also played roles of an observer, communicator, coordinator,and consultant in the medical team to help the patient. By providing nursing instruction, we helped the patient pass the whole therapeutic course smoothly, obtain well home care and good social adaptation.
