The purpose of this article was to describe the nursing experience of a young man with chronic disease. For individuals afflicted with chronic diseases subjected to the development of potential complications, the concept of chronic disease self-management is especially important. Through interviews, communication, and observation from an integrated physiological, psychological, spiritual, and social perspective, the author revealed two important nursing issues for that patient, “anxiety” and “overweight”. Subsequent follow-up of the patient was conducted through outpatient visits, telephone interviews, emails, and text messages. Based on the self-regulation theory, the author acted as a health care provider who led the patient and his family through the process of continuous self-management as well as provided psychological support for alleviating anxiety, modifying their behaviors, and enhancing self-efficacy. The results of the present study suggest that the quality of care for patients with chronic diseases can be improved by continuous nursing intervention based on the concept of chronic disease self-management through strategic use of information technologies. |
