運用TRM建置集中檢疫所執行成效－－臺中醫院推動經驗

Using TRM to Build a Centralized Quarantine Station Implementation Results-Taichung Hospital Promotion Experience

2019年底爆發嚴重特殊傳染性肺炎(COVID-19)迅速蔓延全球。台灣中央疫情指揮中心除採行嚴格的邊界管制並制定檢疫制度，為安置檢疫民眾，徵用公立訓練所、宿舍、大型旅館、飯店等設立為集中檢疫所。衛生福利部臺中醫院於2020年初成立中區第一家集中檢疫所，收住第一批武漢包機返台國人。陸續再建置共六家集中檢疫所，讓宿舍或飯店瞬間轉變為符合防疫感染管制動線，有紀律高效率的「集中檢疫所」，協助入境旅客(包機、外籍移工、學生等)渡過檢疫隔離期、收治包含北病南送的隔離居民及確診民眾。本文在於描述衛生福利部臺中醫院運用團隊資源管理(team resource management, TRM)模式引領醫療、警政單位、衛生局、在地單位及民間飯店業者進行跨團隊整合，經共識後擬訂作業標準、明訂各組職責與各項檢疫流程。再依據標準指引進行現場督查、教育訓練，進而提升各組值勤人員跨業知能作業執行正確率。以公醫任務為核心，TRM為工具，醫院指揮官發揮TRM-Leadership角色，以策劃、執行及監督角色全面推廣防疫作為，凝聚防疫共識，彼此分工合作各司其職，所有面臨的任務，都挑戰著值勤人員的集中力及耐受力，全院同仁義無反顧正面迎戰堅持至今，至今收治12,000人(確診者達8,200人)，維持集中檢疫所「0群聚、0死亡」。

At the end of 2019, severe special infectious pneumonia (COVID-19) broke out and spread rapidly around the world. In addition to adopting strict border control and formulating a quarantine system, Taiwan's Central Epidemic Command Center has requisitioned public training centers, dormitories, large hotels, restaurants, etc. to set up centralized quarantine centers for the placement of quarantined people. The Taichung Hospital of the Ministry of Health and Welfare established the first centralized quarantine station in Central District in early 2020, and received the first batch of Chinese returning to Taiwan on a chartered flight from Wuhan. A total of six centralized quarantine stations were built successively, so that the dormitory or hotel can be instantly transformed into a disciplined and efficient ''centralized quarantine station'' that conforms to the epidemic prevention and infection control line, and assists inbound passengers (chartered flights, foreign migrant workers, students, etc.) Quarantine and isolation period, admission and treatment include isolated residents and confirmed people sent from north to south. This article is to describe the use of team resource management (TRM) model in Taichung Hospital of the Ministry of Health and Welfare to lead the cross-team integration of medical, police units, health bureaus, local units, and private hotel operators. Define the responsibilities of each group and various quarantine procedures. Then carry out on-site supervision, education and training according to the standard guidelines, and then improve the correct rate of execution of cross-industry knowledge and ability operations of the personnel on duty in each group. With public medical tasks as the core and TRM as a tool, hospital commanders play the role of TRM-Leadership to comprehensively promote epidemic prevention with the role of planning, execution and supervision, build a consensus on epidemic prevention, and cooperate with each other to perform their duties. Challenging the concentration and endurance of the staff on duty, all colleagues in the hospital have continued to fight head-on without hesitation. So far, 12,000 people have been treated (8,200 confirmed cases), and the centralized quarantine center has maintained ''0 clusters and 0 deaths.''
