術中意外發現的，卵巢惡性顆粒細胞腫瘤：病例報告及文獻回顧

Incidental finding of ovarian granulosa cell tumor during lapaoscopic surgery: a case report and review of literature review

隨著微創手術的推廣及臨床醫師的技術進步，越來越多病人在需要外科治療時選擇接受微創手術，然而在面對病灶可能有惡性的風險時、或是術中冰凍切片報告為惡性時，該如何決定後續的治療方式，都需要醫生在術前和病人的解釋及術中和家屬的討論，共同做出最適合的決策。藉由以下的病例及文獻回顧，討論術中意外發現卵巢惡性腫瘤之議題。

Laparoscopic surgery provides benefits including reduced postoperative wound pain, faster recovery, and shorter hospital stays. However, the use of laparoscopic surgery for resection or ovarian cancer appears to be limited because not easy for full exploration of abdominal cavity to search tumor implants. It is also associated with increased risk of ovarian cyst rupture which causes tumor seeding. Therefore, management of unexpected ovarian malignancy during operation should be fully explained and discussed with the patient. We report a case of 74-year-old woman who was incidentally diagnosed with ovarian cancer during laparoscopic surgery and final pathology reprot revealed ovarian granulosa cell tumor. Therefore, we systemically reviewed the clinical case and options of treatment.
