Systematic retroperitoneal lymphadenectomy is necessary to accurately determine the stages for ovarian cancer. According to the current guidelines of the International Federation of Gynecology and Obstetrics (FIGO), the removal of both pelvic and paraaortic lymph nodes during surgery is still a recommended procedure for apparent early stage (FIGO I and II) epithelial ovarian cancers. However, the therapeutic significance of systematic lymphadenectomy for patients with early-stage ovarian cancer is a matter of great debate. On the other hand, systematic lymphadenectomy may increase surgical morbidity, although it is necessary for accurate clinical staging and has diagnostic valus. After literature review, it seems to be state that systematic lymphadenectomy in patients with early ovarian is not necessary, and the presence of macroscopic retroperitoneal retroperitoneal lymph nodes should be resected as part of cytoreductive surgery. We present an extensive review of the available literature on the matter, hoping to provide some insight into the true need for such a procedure. |
