無性經驗的女性發現輸卵管卵巢膿瘍合併子宮內膜異位

A case report: endometriosis combined with tubo-ovarian abscess in a non-sexually active woman

骨盆腔發炎通常被認為是經過性行為而傳播的疾病，而上行性細菌的感染而後導致 了輸卵管卵巢膿瘍，於是很少發生在沒有性行為的女性身上。但近期的研究發現仍 有無性行為的女性有上述診斷，危險因子除了較差的衛生習慣、生理構造異常、接 受婦科手術、闌尾炎或憩室炎。而近期子宮內膜異位也被認為與輸卵管卵巢膿瘍相 關，顯著影響發生率、治療天數及治療失敗率。以下呈現一位於本院治療輸卵管卵 巢膿瘍的女性，同時回顧輸卵管卵巢膿瘍發生在無性行為的女性，以及與子宮內膜 異位的關係。

The relationship between endometriosis and tubo-ovarian abscess was mention ed in previous stuidies. When it comes to a patient diagnosis with endometriosis and pelvic inflammatory disease, there is a chance of the endometriosis further progressed into tubo-ovarian abscess. As we know the women who is nonsexually active are rarely diagnosed with pelvic inflammatory disease. But in the female with lower abdominal pain and fever, combined the circumstance of poor hygiene, previous gynecology surgery, genitourinary tract anomaly, appendititis or diverticulitis, and recurrent urinary tract infection, we still need to put tubo-ovarian abscess in our differential diagnosis, even though the pa-tient is a virgin.
