The role of chemotherapy in early endometrial cancer with lymphovascular space invasion (LVSI)

淋巴血管侵犯的定義，為在惡性腫瘤外圍的淋巴管和血管的內皮層中有存活的腫瘤 細胞。子宮內膜癌是婦科最常見惡性腫瘤，5 年存活率高，但仍有少數的早期患者 會有復發。復發因子包括年長、分化不良、深度肌肉層侵犯、非類子宮內膜癌之癌 細胞等，但有淋巴及血管侵犯被視為是復發的重要危險因子，即使在第 I 期低危險 患者仍有其風險。早期淋巴及血管轉移的子宮內膜癌患者，如手術後輔以化學或合 併放射線治療，顯示有改善。

Lymphovascular space invasion has been defined as the presence of viable tumor cells in endothelial lined channels, outside the bulk of the invasive tumor. Endometrial cancer is the most common gynecologic malignancy, in which 5-year disease-specific survival rate is high. However, it has been estimated that somewhat cases to recur within early-stage, low-risk endometriod endometrial adenocarcinoma. Although including other risk factor for recurrence, such as age, grade, depth of myometrial invasion, tumor size, and non-endometrioid cell type, LVSI is still a strong independent risk factor for the development of pelvic and distant recurrences even in low risk stage I endometrial cancer. Therefore, chemotherapy alone or in combination with radiotherapy seems to be beneficial for positive LVSI early stage endometrial cancer.
