一位精神分裂症患者自我照顧能力缺失之護理經驗

運用Orem理論照護一位精神分裂症患者在急性期症狀嚴重惡化，自我照顧能力缺失之護理經驗。自2006年11月4日至2006年12月28日，其護理問題為自我照顧能力缺失、知識缺失（針對個案及家屬）和社交互動障礙。運用Orem的自我照顧理論、自我照顧缺失理論及護理系統理論，提供完全代償性、部份代償性護理措施，協助個案完成自我照顧；應用支持性教育性護理系統教導個案自我照顧，提升個案的能力，鼓勵家屬共同參與及學習。出院準備計畫包括指導個案及家屬做好自我照顧、遵從服藥及居家照顧。個案在出院前可完全地自我照顧，但社交互動問題仍存；出院後電訪追蹤個案返診，提供家屬支持以協助持續居家照顧，遂使個案能重返家庭及回歸社會。

This abstract used the Orem theory to assess a schizophrenia patient’s self-care deficit. The study was done from November 4 to December 28, 2006, measuring nursing problem like selfcare deficit, knowledge deficit, social interaction deficit. The totally compensatory system and partly compensatory system, that accepted care and assistance from nurses were provided. There was a supportive-education system to teach the knowledge. It promoted the patient's ability, and encouraged the family to participate in and learn. The initial discharge planning afterward. The patient could complete selfcare but still had an interaction deficit. But we used telephone to follow up, and give support and home care to help the patient return family and society.
