降低婦產科病房退藥時間之改善專案

Reducing the time for Returning Medication in obstetrics and gynecology wards

退藥攸關病人用藥安全及醫療資源浪費等問題，「提升用藥安全」是醫策會病人安全目標；住院病人因病情改變，致已配發藥物須再度退回藥局，耗費護理人員時間處理退藥事宜，間接擠壓照護服務的人力與時間。退藥步驟繁瑣、流程花費時間長是本專案主要的問題。透過跨團隊合作簡化退藥流程、降低退藥作業時間，提升護理人員對退藥作業系統的滿意度。結果如下：1.新增條碼退藥作業系統，縮簡操作步驟由8個縮簡為3個；2.單筆退藥平均時間由25.2秒降至10.7秒，減少14.5秒；3.護理師對新條碼退藥流程滿意度由5.7分提升為9.0分。期望透過此次的團隊合作能創造更優質的正向護理執業環境。

Patient medication safety is crucial. Changes in the patients’ conditions or doctors’instructions lead to a large amount of medicine being returned to pharmacies. Subsequently,nursing staff spend a large amount of time managing treatment withdrawal, which indirectlyreduces the time available for providing nursing services.The process for returning medicine is complicated and time consuming. Therefore, thisproject aimed to simplify the withdrawal process through cross-team cooperation, therebyreducing the time of withdrawal from medical treatment withdrawal and improving nursing staffsatisfaction regarding the drug withdrawal operation system. The results were as follows: 1. Anew barcode withdrawal operation system reduced operation steps (from 8 steps to 3); 2. Theaverage time for a single withdrawal was reduced from 25.2 seconds to 10.7 seconds, representinga decrease of 14.5 seconds. The nursing staff satisfaction regarding the new barcode withdrawalprocess increased from 5.7 to 9.0. This teamwork could help create a better environment forpositive care practice.
