Postoperative ileus is a common complication of malignant gynecologic operation, leading to increased morbility and hospital stay. Perioperative strategies employed to prevent postoperative ileus include avoidance of preoperative fasting and mechanical bowel preparation, use of epidural or spinal regional anesthetics, modification of pain management strategies to limit opioid use, gum chewing and early ambulation. When these strategies are used in combination as part of a fast-track surgery, there is a significant decrease in time to return of normal bowel function and a shortened hospital stay. |
