門診醫師對電子病歷系統使用重要性認知與滿意度之研究――以高雄某市立醫院為例

Research on the importance of awareness and satisfaction of physicians use electronic medical records system - a Kaohsiung City Hospital Case

根據衛生福利部民國103年底統計，至今已有363家醫院宣告實施電子病歷，占全國總家數73.6%。過去相關研究電子病歷系統主要使用者為醫師，電子病歷系統若能協助醫師診療與改善資料處理流程等並促進工作效率與品質，將可提升醫師使用之滿意度。因此，透過醫師的使用方能追蹤與驗證其使用效益。而系統品質、資訊品質和使用者滿意度是組織導入資訊科技成功與否的前因。本研究結合DeLone & Mclean（2003）之資訊系統成功模式，透過重要性―績效分析法（Importance-Performance Analysis; IPA），以第四象限（加強改善重點區）做討論，研究對象為高雄某市立醫院目前使用電子病歷系統之門診醫師，進行結構式問卷調查，回收有效樣本50份問卷，有效回收率為71.43%。本研究結果發現贊成使用電子病歷系統占96.0%，但目前使用上有困難者占58.0%，依據IPA第四象限結果，在「系統品質」構面之優先改善項目為系統「易用性」、「反應速度」及「穩定性」，與「資訊品質」構面為「電子病歷系統能提供完整正確即時的病患就醫與照護資料」為全樣本中較為重視但滿意度偏低之項目；而再延伸探索不同分群者特性對電子病歷系統使用期望程度與實際感受之差異及需求。本研究結果未來可提供相關單位在建置電子病歷系統之改善方針。

According to the Ministry of health and welfare statistics in 103 year, at present, 363 hospitals declared the implementation of electronic medical records, 73.6% of the total number of nationwide. This study combined with DeLone & Mclean (2003) Informational System Success Model, through Importance-Performance Analysis, in the fourth quadrant (strengthening improve focus area) do discuss, object of study is Kaohsiung Municipal of a Hospital there physicians use electronic medical records system, Conduct Structured questionnaire, effective samples recovered 50 questionnaires, the effective rate of 71.43%. The results of this study found in favor of the use of electronic medical records system accounted for 96.0%, but the use of the current difficult accounted for 58.0 percent, IPA based on the results of the fourth quadrant, priority improvement projects “quality system” facet of the system “ease of use”, “response rate” and “stability”, And “information quality” facets to “electronic medical record system can provide complete and accurate real-time patient data for medical treatment and care,” but more emphasis on the full sample of projects for the low satisfaction. And then extended by exploring different grouping of characteristics and differences in the use of electronic medical records system needs and expectations of the extent of the actual experience. Future results of this study may provide the relevant units of the electronic medical record system to improve the build in approach.
