協助一位腦性麻痺患童母親渡過依賴呼吸器照護之調適過程

Helping the Adaptation Process of a Mother who Has a Child with Cerebral Palsy Depending on a Ventilator

面對腦性麻痺的女兒因餵食嗆傷，造成吸入性肺炎併發呼吸衰竭，需要長期依賴呼吸器到接受氣切手術的歷程對主要照顧者而言是莫大的衝擊。本文乃運用羅氏適應模式來評估且協助一位母親面臨女兒依賴呼吸器時的情緒反應與適應過程。護理期間將筆者與母親的互動，以會談、觀察、病史收集等方式整理分析，發現母親在女兒診斷為呼吸衰竭併呼吸器依賴時會產生睡眠型態改變、焦慮、罪惡感、親職角色衝突等情緒反應，筆者依個案的反應及需求提供適當的照護，陪伴個案順利渡過此調適過程。

It was a great impact on the caregiver, whose daughter with cerebral paralysis got aspiratory pneumonia with complication of respiratory failure because of suffocation during feeding and needed long-term reliance on a ventilator. Using Roy’s adaptation model to assess the mother’s emotional response and adaptive process, the author collected data during interaction with the mother by interview, observation, and medical history collection. The findings showed the mother’s emotional response of changing sleep pattern, anxiety, guilty, and conflict of parental role of the mother when her daughter was diagnosed with respiratory failure and ventilator dependence. The author provided appropriate care based on the child’s reactions and needs and accompanied with the mother through the adaptation period.
