提昇護理人員對下肢骨折病人術後復健護理指導之認知

Improving Nurse’s Knowledge in the Guidance of Rehabilitation Nursing Care for Post Operation Patients with Lower Extremity Fracture

護理人員照護下肢骨折術後病人時，經常著重於傷口護理而忽略術後早期復健運動之重要性，使得病人長期臥床不但無助於復原，更易誘發合併症。因此本專案旨在提昇護理人員對下肢骨折病人術後復健護理指導認知之成效。以結構式問卷調查南部某區域醫院骨科病房之專科護理人員，有關下肢骨折術後復健護理指導認知成績僅61.4分。分析原因為缺乏適當的下肢骨折術後復健護理指導流程指引與手冊，及未持續舉辦下肢骨折術後復健護理指導相關在職教育訓練。針對上述問題擬定改善對策及實施包括：建立完整的下肢骨折術後復健護理指導流程指引及手冊，並定期舉辦相關在職教育訓練。經改善方案實施後，護理人員對下肢骨折術後復健護理指導認知平均得分由61.4分提升為90.4分。藉由此專案推動，強化護理人員落實復健護理指導認知之正確性，協助病人能盡快恢復最大肢體活動功能及生活品質。

It is common that nurses usually focus on wound care but ignore the importance of early rehabilitation exercise while taking care of lower leg fracture patients. Patients only laying down on beds cannot help their rehabilitation and could easily introduce complications. The purpose of this project was to improve nurses’ knowledge about the guidance of rehabilitation nursing care for post operation patients with lower leg fracture. We applied a structured questionnaire to evaluate nurses’ knowledge about the guidance of rehabilitation nursing care for patients in an Orthopedic ward of a regional hospital in southern Taiwan and the score was only 61.4 points. The results showed that nurses lacked of adequate pamphlet of the rehabilitation nursing care guidelines and lacked regular and constant continuing education training. To solve the problems, we developed a booklet about the assessment of rehabilitation nursing care and improved continuing education for nurses regularly. The results showed that the test score improved from 61.4 to 90.4 points. The project effectively improves the accuracy of nurses’ rehabilitation nursing care guidance. Patients’ functional restoration in extremities and quality of life can be improved accordingly.
