運用水凝膠敷料（Aquacel）於一位乳癌合併蕈狀潰瘍傷口病患的護理經驗

Experience of Applying Hydrofibre Dressing (Aquacel) for a Breast Cancer Patient with Fungating Wound

由於腫瘤傷口易出血感染、疼痛、分泌物多、惡臭，不易處理對護理人員是極大的挑戰。由於生物科技的進步，水凝膠敷料(Aquacel)被發現能高度吸收傷口分泌物，易清除、換藥時不會造成傷口二度傷害，因此適用於腫瘤傷口。本文係探討一位乳癌合併惡性蕈狀潰瘍傷口病患的護理經驗，出現組織完整性受損的護理問題外，腫瘤傷口引發換藥的疼痛、出血、焦慮、身體心像改變等，運用水凝膠敷料(Aquacel)於腫瘤傷口護理，並控制傷口出血、分泌物、惡臭及疼痛，結合整個醫療團隊給予整體性及持續性的治療及照護，心理問題方面，使病患獲得生命尊嚴、提高生活品質，促使病患對於疾病治療更具信心，期望能與護理同仁分享並作為臨床照護的參考。

Because tumor wounds are often confronted with onset of bleeding, pain, excessive exudates, and odour, the problems appear to be great challenges to nurses. Hydrofibre dressing (Aquacel), a biotechnology product, has been found to possess the advantages of high absorption of exudate, easy to clean, and no secondary damage to the wound when changing the dressing, which are ideal for caring tumor wounds. This study discussed the nursing experience of applying hydrofibre dressing (Aquacel) for caring the fungating wound of a breast cancer patient. Comprehensive and continuous treatment and care were provided by the medical team. This patient not only regained self-esteem but also developed confidence in medical treatment. The quality of life of the patient has greatly improved by the newlydeveloped technology. The study provided a reference to nursing staff for caring similar patients.
