應用羅氏理論照顧一位上顎癌病患之護理經驗

Applying Roy’s Adaptation Model in Caring a Patient with Palatal Cancer

本文應用羅氏適應模式，照顧一位罹患上顎癌術後因傷口感染併發慢性骨髓炎之病患，筆者自91年7月1日至91年8月20日護理期間運用Roy理論的第一層次及第二層次評估，並加以確立問題，發現個案有急性疼痛、組織完整性受損、身體心像紊亂等護理健康問題。於護理過程中，應用非藥物性處置減輕疼痛，監測傷口及感染情形，並強化病患正向行為與態度，協助個案面對疾病病程的各種不同之刺激，促進病患的適應反應，期望藉此護理經驗的分享，能提供臨床護理人員照顧此類病患之參考。

This paper reported the nursing experience of a palatal cancer patient, who had wound infection complicated with chronic osteomylitis during postoperative period. The first and second level assessments in Roy’s theory were used to assess and identify nursing problems and set goals and care plans. There were three major problems, including acute pain, impaired tissue integrity, and disturbance in body image. During the care period, primary nurses applied non-pharmacological management to reduce pain, control wound infection, and enhance patient’s positive behavior and attitude to manage the different stimulus from the progress of disease and promote her adaptation reaction. This report provides nurses with a reference in caring similar patients in the future.
