運用Orem理論照顧一位食道灼傷患者於食道重建術後之經驗

The Postoperative Nursing Experience of an Esophageal Injuried Patient Receiving Reconstruction: An Application of Orem's Theory

本文探討一位食道灼傷患者接受食道重建術後之護理經驗，筆者在91年6月23日至91年7月13日護理期間透過觀察、訪談、傾聽及身體評估等方式，依據Orem理論加以確立問題及目標，發現個案有疼痛、營養狀態改變少於身體需要及高危險性感染等護理健康問題，在護理過程中筆者運用支持及教育性、部份代償性、完全代償性的護理系統，提供個案支持與鼓勵，提昇其自我照顧能力，期望藉此護理經驗之分享，能提供護理人員臨床照護之參考。

This paper reported the postoperative nursing experience of an patient receiving esophageal reconstruction. The Orem's theory was used to identify nursing problems and set goals for caring plan. There were three major nursing problems, including pain, nutrition alteration: less than body requirements, and high risk for infection. Supportive and education programs, partial and complete compensation were provided to improve the patient's self-care ability. This experience provided nursing staffs with a reference for caring similar patients in the future.
