背景與目的:臺灣住宿式機構使用率高,機構的照顧品質為不可忽視的重要指標。照服員為長照機構最主要的照顧人力,機構照顧品質優劣與機構照服員的能力息息相關,因此,建立機構照服員核心能力量表有其重要性,故本研究目的為瞭解機構照服員的能力內涵,以發展機構照服員核心能力量表(Core-competency Institutional Nurse Aidescale, CINA)。方法:分為三個步驟。一、文獻回顧:發展初版的CINA;二、專家審核:修訂CINA並檢驗內容效度指標(Content Validity Index, CVI);三、認知訪談:建立CINA最終版和確定表面效度(face validity)。結果:本研究根據文獻回顧發展初版CINA,執行三回專家審核,第三回合之CVI值皆為1,具有良好的內容效度,並執行二回認知訪談,建立良好的表面效度,完成CINA最終版包含八面向(共74題):身體照顧、生活照顧、緊急狀況處理、心理支持、人際溝通、活動促進、團隊合作及職業倫理。結論:本研究CINA具有良好的內容效度與表面效度,能使機構照服員瞭解其職務之核心能力,並提供機構管理者訂定進階教育訓練,改善機構照顧品質。 |

Background: The utilization rate of services in residential institutions is high in Taiwan. The quality of care provided by institutions is an important indicator that cannot be ignored. Nurse aides are the most important caregiversin long-term care institutions. The quality of institutional care is closely related to the ability of institutional nurse aides. Therefore, it is important to establish a core competency scale for institutional nurse aides. The purpose of this study was to understand the competency of institutional nurse aides and to develop the Core competency Institutional Nurse Aide scale (CINA). Methods: There were three steps: (1) Literature review for developing the first version of the CINA; (2) Expert review for revising the CINA and estimating content validity(CVI); and (3) Cognitive interview for verifying the final version of the CINA and determining face validity. Results: We developed the first version of the CINA based on a literature review, and carried out three rounds of expert reviews. The CVI values were 1 in the third round of expert reviews, demonstrating good content validity. Moreover, two rounds of cognitive interviews were administered and good face validity was established. The final version of the CINA consisted of 74 items with 8 domains: physical care, daily care, emergency management, psychological support, interpersonal communication, activity facilitation, teamwork cooperation, and occupational ethics. Conclusions: The CINA had good content validity and face validity, which could enable institutional nurse aides to understand the core competencies required for their duties, and provide institutional managers with advanced on-the-job education and training, which would further improve the quality of care in institutions. |
