Individuals with human immunodeficiency virus (HIV) infection may be at increased risk of complications and mortality from severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection, but clinical data is limited. This study reports the case of a previously healthy 38-year-old male who had received four doses of SARS-CoV-2 vaccine. He presented with dyspnea and was diagnosed with SARS-CoV-2 pneumonia. Despite treatment with antibiotics and antiviral agents, his respiratory status deteriorated and was complicated by right-sided pneumothorax and respiratory failure, so a chest tube was inserted, mechanical ventilation was initiated and extracorporeal membrane oxygenation (ECMO) was performed. Due to progressive pneumonia, underlying immunodeficiency was suspected. HIV testing revealed a HIV-1 infection with a CD4+ T-cell count of 27 cells/mm3. Despite prompt initiation of antiretroviral therapy (ART), the patient died of multiorgan failure. This case underscores the importance of HIV screening for young patients with severe SARS-CoV-2 infection and no known risk factors, in order to allow early diagnosis and reduce complications and mortality. |
