A Project to Improve the Rate of Early Oral Intake Within 24 Hours After Liver Cancer Surgery

本專案為提升肝癌術後病人24小時內及早進食率。根據統計結果手術後提早進食率僅15.3％，平均進食時間為52.3小時，護理人員對照護認知率為66.6％。專案小組調查發現指導內容不一致、無標準流程、醫囑不明確及缺乏定期監測等問題。遂推動術後加速康復（Enhanced Recovery After Surgery，ERAS）課程、製作衛教影片、制定作業流程與新增醫囑套餐，並設立照護監測機制。專案執行11個月後，進食率提升至90.9％，照護認知率提升至93.7％，平均住院日數縮短三天。建議肝癌術後病人可轉介至營養門診，持續進行個別化飲食指導及照護追蹤，以維持改善成效並強化護理專業品質。

This project aimed to improve the early oral intake rate within 24 hours after liver cancer surgery. According to initial data, only 15.3% of patients began eating within 24 hours postoperatively, with an average initiation time of 52.3 hours. The nursing staff's awareness rate regarding postoperative nutritional care was 66.6%. The project team identified several issues, including inconsistent patient education, lack of standardized procedures, unclear medical orders, and absence of regular monitoring. To address these, the team implemented Enhanced Recovery After Surgery (ERAS) training, produced educational videos, established standardized care protocols, introduced preset medical order sets, and developed a monitoring system. After 11 months of implementation, the early oral intake rate increased to 90.9%, nursing staff awareness rose to 93.7%, and the average length of hospital stay was reduced by three days. It is recommended that postoperative liver cancer patients be referred to nutritional outpatient services for individualized dietary counseling and ongoing care follow-up to sustain improvements and enhance nursing care quality.
