Background: Forensic psychiatry in Taiwan is under the influence of three factors, i.e., mental health care, legal authority, and public safety. Over the past decade, legislative reforms and evolving clinical practices have shaped how involuntary psychiatric treatment is delivered and supervised. Here, we are reviewing recent developments in forensic psychiatric care in Taiwan with particular attention to continuity of care across legal and clinical systems. Methods: In this review, we are covering legislative developments, service organization changes, clinical practice innovations, and descriptive service-level information reported between about 2016 and 2025. Relevant topics include involuntary hospitalization under mental health legislation, court-ordered custodial disposition, forensic psychiatric evaluation, interim placement during criminal proceedings, and court-associated addiction treatment programs. Results: Recent reforms have strengthened judicial oversight in involuntary psychiatric care while expanding collaboration between mental health services and legal authorities. Clinical advances, including the increasing use of long-acting injectable antipsychotics, have contributed to improved symptom stabilization among individuals with severe mental illness. Emerging service models have demonstrated enhanced continuity between inpatient treatment and community-based follow-up. Experiences from deferred prosecution with addiction treatment further illustrate how structured psychiatric–judicial collaboration may support sustained care beyond institutional settings. Conclusion: Current developments suggest that the effectiveness of forensic psychiatric care in Taiwan depends less on legal classification alone than on the availability of coordinated, continuous treatment across clinical and judicial systems. Continued development of specialized services, integrated data systems, and community follow-up mechanisms will be essential to address the growing complexity of court-supervised psychiatric care.
