2022年桃園市移工本土急性病毒性A型肝炎群聚事件調查與防治作為

Investigation and Control of a Hepatitis A Outbreak among Migrant Workers, Taoyuan City, 2022

2022年1至3月桃園市共出現29例泰籍移工A型肝炎病例，發病前2個月內查無出境紀錄，皆因症就醫通報。經逐案疫調，確定病例潛伏期及可傳染期均無交集、發病日分佈呈現單一波峰突起，推測為食因性傳播。病例對照研究結果顯示傳播途徑可能與攝入受汙染之生牛肉有關（勝算比3.85，95%信賴區間1.47–10.04）。藉由跨部門合作，即時完成疫情調查、接觸者暴露後預防處置、多元管道衛教宣導等防治作為。為阻止類似群聚事件，建議於移工入境前加強宣導飲食衛生，避免生飲、生食，並鼓勵接種A型肝炎疫苗，以降低因不同生活、飲食習慣，於高風險族群間造成群突發之風險。

During January 1 to March 31, 2022, 29 hepatitis A cases were reported among Thai workers in Taoyuan City. None had a travel history within two months. Epidemiological investigations revealed no overlap in infectious periods, with a single peak in onset dates, suggesting foodborne transmission. We conducted an outbreak investigation, including a case-control study, which showed that the consumption of raw beef (odds ratio: 3.85, 95% confidence interval: 1.47–10.04) was the most probable way of infection. Through cross-departmental collaboration, prevention and control actions such as epidemic investigation, post-exposure prophylaxis for contacts, and diversified health education and promotion were implemented. To prevent similar outbreaks, we recommend enhancing education on dietary hygiene, discouraging the consumption of raw food, and promoting hepatitis A vaccination for foreign workers prior to their entry into Taiwan.
