屏東縣社區護理師高齡友善照護之經驗──焦點團體研究

The Experiences of Community Nurses Providing Age-Friendly Care in Pingtung County: A Focus Group Study

背景：屏東縣幅員廣闊，涵蓋多元文化族群，期望強化地緣特性及跨文化溝通，營造以人為本的高齡樂智友善照護模式。
目的：本研究旨在從社區護理師的觀點，探討社區長者（含失智）的護理經驗，並確定其核心訓練的需求。
方法：採焦點團體研究法，招募社區護理師，參加焦點團體座談，每組成員10−12人，每個場次約進行120分鐘，訪談資料經錄音謄寫後進行內容分析，及嚴謹度的控制。
結果：2023年8月9日共舉辦2場次，共有12位護理主管、13位護理師參與研究。研究結果呈現共有六個主題：(1)培訓跨文化社區照護及系統性照護能力；(2)專業能力的修為與提升；(3)提升實證應用能力，激發創新潛能；(4)推廣實證應用、確認自我定位；(5)領導管理翻轉、組織再造；(6)優化失智症之照護、營造友善氛圍。
結論：社區護理師是維護全民健康的重要角色，面對高齡化、多元族群文化、地緣特性與失智症等特殊需求，提供符合跨文化特性之系統性照護、提升實證應用能力及強化領導管理是社區高齡友善及失智照護的重要關鍵。

Background: Pingtung is a county in Taiwan that covers a large area and a population marked by significant cultural diversity. Healthcare programs in the county must take geographical and cultural needs into consideration to create a person-centered, friendly-care model for the county’s older adult residents.
Purpose: This study was designed to explore the experiences of community nurses in Pingtung County caring for adults in the community and identify related core competency needs.
Methods: A focus group research method was used, and community nurses were recruited to participate in focus group discussions. Each focus group included 10 to 12 nurses, and each session lasted about 120 minutes. The interview data were recorded, transcribed, content analyzed, and rigorously controlled.
Results: The two discussion sessions organized for this study were both held on August 9, 2012, with a total of 12 head nurses and 13 registered nurses participating. The researchers identified the following six themes: 1. Training of cross-cultural community care and systemic care capabilities; 2. Cultivation and improvement of professional capabilities; 3. Improving capabilities of evidence implementation and innovative ideas; 4. Promoting evidence implementation and self-identity; 5. Leadership management transformation and organizational reengineering; 6. Optimizing dementia care and creating a more patient-friendly atmosphere.
Conclusions: Community nurses play an important role in maintaining and improving public health. Special needs such as aging, multiculturalism, geographical characteristics, and dementia requires that systematic care provided be sensitive to cultural characteristics, work to improve evidence-based application capabilities, and strengthen leadership and management.
