2020年東部某矯正機關結核病聚集事件調查

Investigation of Tuberculosis Outbreak in a Prison in the Eastern Taiwan, 2020

2020年10月1日至11月18日東部某矯正機關陸續通報3名收容人確診結核病，衛生單位啟動疫情調查發現分屬不同房舍及作業場區，結核病菌株為同基因型別，確定為結核病群聚事件。後續有4名接觸者發病確診，其中2名為未被匡列之高傳染力個案。經專家會議建議擴大接觸者匡列、運用資訊系統勾稽移（出）監名冊、每半年1次接觸者檢查，監測至2021年10月共通報10名，確診7名，同基因型別6名。接觸者匡列717名，4次胸部X光檢查完成率為55.2%–98.2%，截至2022年4月未再新增同基因型別個案。本事件經各專家委員、醫療公衛人員及矯正署等跨單位合作，順利將疫情控制住，落實接觸者匡列及潛伏結核感染篩檢與治療，才能降低矯正機關未來發生群聚事件之風險。

From October 1 to November 18, 2020, three inmates incarcerated in eastern Taiwan were diagnosed with tuberculosis (TB). An epidemiological investigation revealed that the three inmates lived in different living quarters and worked in different workplaces. This event was confirmed to be a tuberculosis outbreak after the TB strains were found to be of the same genotype. Subsequently, four contacts were diagnosed with the disease; two of whom were highly infectious. The expert meeting recommended expanding contact tracing efforts, utilizing the prison information system to verify transfer records, and conducting contact examinations every six months. As of October 2021, of the 10 cases that had been reported, 7 had been confirmed, and 6 cases had the same genotype. There were 717 contacts, and the completion rates of the four cycles of chest X-ray examinations were 55.2%–98.2%. As of April 2022, no more cases with the same genotype had been reported. This outbreak had been successfully controlled through inter-departmental cooperation, effectively mitigating and reducing the risk of future outbreaks in correctional institutions.
