一位由尿道寡養桿菌感染導致菌血症與肺炎的案例

A case of Bacteremia and Pneumonia Caused by Oligella urethralis

尿道寡養桿菌（Oligella urethralis）被認為是人類生殖泌尿道的共生菌之一，其致病力與毒性均不高。尿道寡養桿菌導致的感染並不常見，偶爾會造成泌尿道感染。在免疫低下或是年紀較大的病人，仍然可以導致嚴重的感染。我們報告的案例是一位93歲女性，其病史有糖尿病、高血壓、腎功能不全，以及曾有輕微中風。因為咳嗽、呼吸困難與發燒而住院，血液培養結果鑑定為尿道寡養桿菌。在更改抗生素為cefmetazole治療下，其臨床症狀獲得緩解。由於尿道寡養桿菌感染是罕見的，並且此細菌在培養時生長緩慢，臨床上經常會有被忽略的可能。因此，更進一步的研究並優化細菌鑑定的方法是需要的。

Oligella urethralis is a commensal organism predominantly isolated from the genitourinary tract, known to cause not only urinary tract infections but also extragenitourinary infections. Herein, we report the case of a 93-year-old female with multiple underlying diseases who developed bacteremia and pneumonia following a urinary tract infection episode. The isolate from the blood specimen was identified as O. urethralis. The patient showed an excellent response to cefmetazole treatment, and the infection resolved with an uneventful stay afterward. Due to its slow growth in nature, this bacterium should be carefully examined and recognized by microbiology laboratories in routine practice.
