照顧一位上腸繫膜動脈症候群青少年之急診護理經驗

An Emergency Nursing Experience of Caring for an Adolescent Patient with Superior Mesenteric Artery Syndrome

個案為一位16歲青少年，因腹痛未改善而反覆入院，電腦斷層掃描結果為上腸繫膜動脈症候群，個案必須在短時間內面對學校課業壓力、疾病症狀及面對放置鼻胃管過程的焦慮不安，因而引發撰寫此個案報告的動機。提供個案及家屬了解上腸繫膜動脈症候群的疾病相關資訊，緩解急性疼痛，增進居家自我照顧能力。運用QR-Code掃描連結疾病衛教網頁配合影片說明放置鼻胃管減壓之重要性及疾病相關照護，增加個案及案母對疾病照護的認知，協助個案配合治療與照護，鼓勵個案說出因疾病導致的生理不適及面對未知的疾病治療導致的心理壓力。個案也了解如何居家自我監測及預防疾病復發，同儕及家屬皆是個案很重要的心靈支持來源，建議臨床護理人員須了解患者的生理及期盼急診護理人員面對反覆回診的個案，可以更深入評估了解，提供護理照護。

This case is a 16-year-old adolescent who was repeatedly admitted to the hospital due to the lack of the improvement in the abdominal pain. The computed tomography showed that it was the superior mesenteric artery syndrome. The case had to face the school work pressure, disease symptoms and anxiety for the process of the nasogastric tube indwelling within a short time, which has triggered the motivation to write this case report. Provide the information related to the disease of the superior mesenteric artery syndrome for the case and family members to understand, to relieve the acute pain, and to improve the selfcare ability at home. use the QR-Code scanning to link the webpage of the health education on the disease with the videos to explain the importance of the indwelling nasogastric tube decompression and the disease-related care. Increase the knowledge of the case and the mother of the case on the care of the disease. Assist the case to coordinate with the treatment and care, and encourage the case to speak the physical discomfort caused by the disease and the psychological pressure caused by the unknown treatment of the disease. The case also understands how to self-monitor at home and prevent the disease recurrence. The peers and family members are both very important sources of the spiritual support for the case.
