南部某醫學中心精神科病房COVID-19 群聚事件調查

Investigation of a Coronavirus Disease 2019 Outbreak in a Psychiatric Ward of a Medical Center in Southern Taiwan

2022年7月南部某醫學中心精神科病房發生首起病人發燒採檢SARSCoV-2確診，經由疫情調查匡列密切接觸者16位、風險對象42位、擴大採檢對象73位。自7月4日至8月3日，確診累計案例含指標個案共19位，陽性率14.3% (19/132)，包含病人13位(9.1%)、陪病者2位(1.5%)、醫療相關工作人員4位(3.0%)。處置過程因應精神科獨特的醫療挑戰以及社會體制環境的限制，必須快速成立緊急應變小組，進行接觸者匡列採檢、人員出入管制並運用跨團隊資源改善空間限制、改變病室氣流方向以及轉移病人，使得疫情控制於該樓層，並未擴及它區影響全院的營運，該單位也在發生37天之後恢復一般收治。希望藉此事件經驗，作為其他相關醫療機構因應群聚事件發生時之參考。

In July 2022, the first patient with fever in the psychiatric ward of Southern Medical Center was diagnosed with the infection of severe acute respiratory syndrome corona virus 2. After the epidemic investigation, we traced 16 close contacts, 42 risky medical staff members, and 73 screening staff. This outbreak that occurred between July 04, 2022 and August 03, 2022 included 19 cases, with the confirmed rate of 14.3% (19/132), which included 13 patients (9.09%), two family caregivers (1.51%), and four medical staff members (3.03%). The coping process addresses the unique medical challenges of psychiatric wards and the limitations of the social institutional environment. It is important to quickly establish an emergency response team, conduct contact screening and access control, use crossteam resources to improve space constraints, change the airflow direction in the ward, and transfer patients. Finally, we controlled the pandemic in this area, and it did not allow it spread to other areas of the hospital. The operations of the entire hospital were not affected. The unit also resumed general admission 37 days after the outbreak.
