腫瘤科病人跌倒防範改善方案

Project on Reducing Oncology Patient Fall Incidence

本專案之腫瘤科病房於2002年與2003年的病人跌倒率分別為.12％與.11％，均高於全院監測閾值(＜.1％)，分析發現缺乏專科跌倒防範流程與病人及陪伴者對跌倒認知不足為主因；為降低跌倒發生率至閾值以下，制定腫瘤科跌倒防範流程、跌倒危險因子評估表、護理指導記錄單、探視單及收案完整性稽核表等，並製作明顯警告標示、警示鈴鐺與照顧者外出警示牌，加強病人及陪伴者對預防跌倒的認知。專案實施一年後，不但提升病人及陪伴者防跌認知，並能有效降低跌倒發生率，提升護理品質。

Patient falls can increase medical costs, prolong hospitalized days and cause damage.Therefore it is a most important topic. In this ward, the fall incidence was higher than the threshold in the years of 2002 and 2003.Through the intervention of this study,we set up the oncology standard operating procedures, built fall risk assessment tools, established the education and visiting sheets,made warning bells,warning billboards and caregivers egress signposts.We also strengthened the patients, caregivers and nursing staffs’ cognition in the high risk group. The result of the study indicates that we not only enhance the patient and caregivers’ concern for fall prevention , but also reduces the fall incidence rate.
