本國當今的精神醫療照護體系，仍以急性住院照護為服務重點，對於出院後的「後續照顧」則尚未有妥善規劃。本文提出「以病患需求為導向之精神病患出院準備服務概念」及舉例探討，期作為精神醫療照護中出院準備服務或社區照護之參考。

In Taiwan, the most important service of psychiatric care system is still located on acute care more than aftercare. A need-led psychiatric discharge care concept was introduced to guide the discharge care and the recommendations for the future implementing and application were also discussed in this article. This is the right time to take more attention on psychiatric discharge care in order to increase psychiatric outpatients?community adaptation, and the need-led psychiatric discharge care concept could be applied in the discharge transitional process.
