危機處置於一位雙極性情感病患之照護經驗

Crisis Intervention : The Experince of Caring a Patient with Bipolar Disorder, Depressjve Episode

本篇為憂鬱型雙極性情感病患者之照護經驗，個案於住院期間遭遇喪偶之痛，筆者以紐曼系統模式評估個案生理、心理、社會文化、發展、靈性等五大層面，發現有高危險性自殺、睡眠型態紊亂、個人缺乏應付能力、家庭動力過程改變及社交互動障礙等問題，除了針對所呈現之問題，給予應有之照顧外，亦應用危機處置，協助個案面對喪偶後之哀傷、子女撫育及債務等問題。護理人員在照顧的過程中，體認到必須善用相關資源，方能協助個案渡過危機，提供個案身、心、社會整體性之照護及促進健康，亦能從中獲致極大的工作滿足感。

In this paper, we report the caring for a patient with bipolar disorder who was in a depressed episode after experiencing spouse's death during his admission to the acute ward. Using Neuman systems Model to assess the patient's physical, psychological, sociocultural, developmental, and spiritual perspectives of health, we found that this patient had such problems as high risk of suicide, sleep pattern disturbance, lack of individual coping ability, change of family dynamic processes, and disability in social interaction. In addition to taking care of these manifested problems properly, we also applied crisis intervention to help the patient deal with the grief of spouse's death, the burden of rearing children, and the stress of financial debt. Nursing staff, in taking care of this patient, realized that utilization of all available resources are mandatory in assisting clients out of the crisis so as to provide a holistic mind-body-cultural care and promote health and achieve great satisfaction in doing the job.
