Diabetes is a typical self-care disease. In order to improve the health of patients, we listened, cared, and cooperated with family members to overcome the patient’s powerlessness. We also used APPs, QR codes, and YouTube videos to provide guidance on diabetes-related care, increase the patient’s willingness to learn disease care, guide the family to accompany the patient, and strengthen the social support system. Through cross-disciplinary cooperation and individual care guidance, the patient learned about self-care. This study recommends nurses to understand more about the needs of health education among patients with chronic diseases, and to provide specific and appropriate guidance to meet the needs of patients and to truly improve their quality of life.
